If you donate €250 or more in a calendar year, Friends of St Luke’s can claim back tax on your donation from Revenue. This means your gift could be worth an additional 45%, helping us continue to support patients and families at St Luke’s.
All you need to do is complete a simple CHY3 tax form and return it to us. It only takes a few minutes, and once completed it can cover your donations for up to five years.

What you need to do
To allow us to claim tax back on your donation, simply:
- Download the CHY3 form here
- Complete and sign it
- Return it to us by post, or email a scanned copy to he***@**************es.ie
If you prefer to complete an annual form instead you can us the CHY4 option
